Plan Descriptions

The Premier Plan Plus (along with the Lumenos Plans below) offer the most comprehensive coverage.  It is offered in deductibles from $500--$10,000.  You may choose a higher major medical deductible and still have your routine services (unlimited office visits, prescriptions- brand & generic, Labs, etc.) with only co-pays.  Premier is offered in two versions: Premier 80 and 100.  Maternity and dental are available as options.  Major medical benefits are paid at 80% or 100% after deductible is met.

 SmartSense Plus Plan offers a lower-cost option.  The plan includes three doctor office visits with only a co-pay (3+ are subject to deductible); full Rx coverage (brand & generic) after a $250 deductible and co-pays.  It is offered in deductibles from $500-$10000.  Major medical benefits are paid at 70% after deductible is met.

Lumenos HSA (Health Savings Account) 

The Lumenos Health Savings Account---HSA plans offer lower health insurance premiums plus a tax benefit. The HSA has two components: (1) A qualified high deductible health insurance policy ($1500--10,000); and  (2) an individual savings account with tax advantages.  You set-up this account with your local bank and use any deposits to pay medical expenses up to your deductible.  These deposits or paid medical expenses are tax deductible (similar to an IRA). 

All Plans include first dollar preventative benefits---no deductible or co-payments for physical exams, routine and other preventive services (PSA, colonoscopy, mammograms, immunizations, etc.)..
